ADLs — Setup & FAQ
Record daily living assistance quickly and consistently. Private to your Circle. No ads, no resale of data.
How it works (simple)
- Identity is synchronized across all healthcare modules.
- Select the shift or time window (e.g., AM / PM / NOC).
- Tap each ADL (bathing, grooming, dressing, toileting, mobility, eating, etc.).
- Choose the assistance level (Independent, Supervision, Limited, Extensive, Total) and add notes if needed.
- Save. Entries appear in the table and can be exported as needed.
- Circle Bot: The AI can now summarize care gaps by analyzing "Refused" ADLs to help identify barriers to care.
Where do the ADL items come from?
- App Defaults: built-in defaults follow common categories (bathing, dressing, grooming, toileting, mobility/transfer, eating, behavior/redirect).
- Custom items: you can add specific ADLs or hide ones you don’t use.
- Templates: optional CSV templates (one ADL per line) can be imported if enabled by your admin.
How do we enter ADLs?
- Manual entry: record assistance levels and notes directly in the modal.
- Synchronization: when opened or renamed in MAR, the patient is updated here automatically.
- Bulk: repeat/clone last daily summary’s ADLs to speed up routine documentation (optional).

Privacy & safety tips
- Chart what’s necessary: keep notes factual and minimal (objective, non-PHI extras are best avoided).
- Follow policy: use the app’s assistance-level definitions and daily schedule.
- Exports: protect exported CSV/PDFs per privacy rules.
- Review: managers can review who charted and when for QA.
Troubleshooting
- Wrong Patient? Change the resident in the MTR/MAR module; ADLs will update instantly.
- Missing ADL item? Add it under Admin > ADLs > Settings (if you have admin rights).
- Wrong shift? Use the date/shift controls at the top to switch windows.
- Network error? Your entries save locally until the connection returns.
